WINNERS: Winning participants will be contacted within 48 hours of contest closure at the provided email address from an official MECA Omaha email address. The winner must respond within 24 hours of contact or their prize will be forfeited.

TO ENTER: The contest may be entered by filling out the above required fields. Any incomplete or invalid fields will be subject to disqualification. The contest may be entered via the designated methods only during the promotion period.  No other methods of entry will be accepted. Limit one (1) entry per person, per e-mail address, per household and Facebook login, regardless of method of entry. Multiple entries received from any person or email address or household in excess of the stated limitation will be void. No purchase is necessary in order to enter.

ELIGIBILITY: The contest is only open to residents of the contiguous United States and District of Columbia where permissible by law, who are at least 18 years old at the time of entry. Employees (including immediate family members (spouse, child, sibling, parent and their respective spouses, regardless of where they reside) and members of the same household, whether or not related) of CHI Health Center Omaha or the promoter are not eligible to participate or win.
